Integrated Primary Care

Through our Integrated Primary Care, our licensed clinicians and care coordinators work with community physicians and other medical staff to support the whole health of you or your child—physical, social and emotional—and to care for you more effectively.

Integrated care looks at symptoms or situations that can put your health at risk or point to other health issues – things like how your child is doing in school or how you are doing on the job, how you’re eating, how active you are, how you or your child is getting along with parents, family or friends, or if you or your child are affected by neighborhood violence or other social stressors.

Integrated care goes beyond either standard medical care or behavioral health services. If emotional issues are affecting your or your child’s physical health, we know how to help.  If your child is feeling anxious or is having a hard time paying attention, we can help with that.  If depression, anxiety or substance use is preventing you or your child from succeeding with daily activities, we can help find answers.  If finding transportation to your child’s appointments is a challenge, we can help with that. If you need academic support or help finding family activities in the community, our care coordinators can connect you to resources.
